I, the parent and/or legal guardian of the child I am enrolling, who is a minor, do hereby authorize my child to participate in all activities related to the Youth Combine Day on November 21, 2026. By buying this
ticket, I, on behalf of myself and the child participating, release Kansas State University and any divisions or subsidiaries thereof, including K-State Athletics, Inc., from liability or responsibility for any injuries and/or damage that may occur affecting myself or my child as a result of Youth Combine Day activities,
and from responsibility or liability for any lost or stolen items during any and all Youth Combine Day activities. I give K-State Athletics permission to use photographs, video and audio recordings, artwork and/or likenesses of myself and/or my child in its publications, electronic communications, social media
and other promotional, marketing and publicity materials used by and representing the Youth Combine Day. I grant this usage without limit and reservation and understand there will be no compensation for this use. I further understand that substitution of non-regeristered people for participants at any of the Youth Combine Day activities is not permitted.
